Important DPS Forms The DPS Parent/Student Handbook offers day-to-day school information that students and parents need. New this year is the calendar segment. We know that parents will find this especially useful, as it allows you space to jot down your own information about particular school happenings. Also included is information regarding promotion standards, testing and behavior standards.

Students who are 18 or older, their parents, and other K-12 schools where students have enrolled may request a student’s record. Fees may be charged for copies of records, including certified copies of transcripts. Make a difference in your community. Download and return one of these to get started.
Student Internet Use (English) (Spanish) If you would like your student to access the Internet for school related activities, download this form.
This forms allows student's photos to be posted on a website. If you do not want to have your student photographed by Durham Public Schools or the media, complete this form.
Schools for All Seasons Intersession Program provides activities for students during the intersession breaks of year-round schools, including day camps, remedial instruction, and enrichment classes.
Kindergarten Health Assessment Form to complete for kindergarten health assessment.
Homebound Form to complete to request homebound service for a student is unable to attend school for health reasons.
