Parent Handbook 2007

The DPS Parent/Student Handbook offers day-to-day school information that students and parents need.  New this year is the calendar segment. We know that parents will find this especially useful, as it allows you space to jot down your own information about particular school happenings. Also included is information regarding promotion standards, testing and behavior standards.

 

Student Data Sheet

 
We want to stay in touch with you about your child’s school experience, too. Fax or send an updated Student Data Sheet to your school if phone numbers, addresses, or emergency contacts have changed.

Medication Request

For students needing to have medication to be administer during school hours

 

Bus Stop Request

Form to request an addition to the stops on a bus route.

 

Absences for Religious Observances

Form to be excused from class for schedule conflict involving religious observance, service or event. 

 

Educational Absences

 Form to be excused from class for schedule conflict involving another educational event or class.

 

Student Records Request

Students who are 18 or older, their parents, and other K-12 schools where students have enrolled may request a student’s record.  Fees may be charged for copies of records, including certified copies of transcripts.

If you would like your student to access the Internet for school related activities, download this form.

 

Student Photo Release

This forms allows student's photos to be posted on a website.

 

Notification of Media Visits

If you do not want to have your student photographed by Durham Public Schools or the media, complete this form.

 

Intersession Registration

Schools for All Seasons Intersession Program provides activities for students during the intersession breaks of year-round schools, including day camps, remedial instruction, and enrichment classes.

 

Kindergarten Health Assessment

Form to complete for kindergarten health assessment.

 

Homebound 

Homebound Form to complete to request homebound service for a student is unable to attend school for health reasons.
